Care Package Guidelines

Our widest-reaching program spans 56 cities in Kentucky & Southern Indiana.

Opal’s signature program is the distribution of more than 8,000 care packages to 100+ nursing homes across Kentucky & Southern Indiana during the holiday season each year. These often include personal care items, activity books, blankets, socks, and other commonly requested items; no religious materials will be distributed, and the care packages are not holiday-specific. Requests will be accepted from facilities served by Opal’s Dream Foundation. New facilities may also apply; only Medicaid residents are eligible to receive the care packages. All requests are subject to review by the Board of Opal’s Dream Foundation.

Care packages for new arrivals who were formerly unhoused and currently receiving Medicaid may be requested throughout the year; these will include personal care items, clothing, and other items subject to availability.

 

Music, Arts, recreation & Culture Program Guidelines

Requests will be accepted from facilities served by Opal’s Dream Foundation. New facilities may also apply, if a majority of residents are Medicaid recipients. All requests are subject to review by the Board of Opal’s Dream Foundation and funds availability. Requests may include:

  • Live musical performances or concert field trips

  • Jukeboxes, karaoke machines, or other media players

  • Musical instruments for group activities

  • Arts & crafts supplies, or other supplies for group activities, or professional group art instruction

  • Field trips to art museums or galleries

  • Social gatherings including parties, cookouts, or outdoor events (on site)

  • Field trips to community or cultural events

Individual request guidelines

Requests will be accepted from and on behalf of individual seniors in long-term care facilities served by Opal’s Dream Foundation. Seniors must be permanent nursing home residents and Medicaid recipients. All requests are subject to review by the Board of Opal’s Dream Foundation and funds availability.

Mobility request guidelines

Medicaid seniors who are permanent nursing home residents are eligible to receive standard motorized wheelchairs; priority may be given based on expected quality of life benefit. This program relies on donations of gently used mobility equipment, and requests are subject to availability and review by the Board of Opal’s Dream Foundation.
